Assessment Intake

  • About

    An intake appointment for an assessment is a session designed to gather essential information about the individual's developmental history, behavior, and current functioning. From this information I will create an individualized plan for the full assessment process. At the end of this appointment we will schedule the remaining appointments necessary to complete the assessment.

  • Scheduling

    To schedule an intake appointment click on the “Schedule Intake Appointment” button below. You will be forwarded to my HIPAA compliant electronic health records system to set up your client profile and schedule your appointment. There is an option for an autism assessment inake or an ADHD assessment intake. If you are wanting an assessment for both please schedule under the autism intake appointment option.

  • Consent & Intake Documents

    Once you shedule your appointment you will receive an email with a link to review and complete the required consent forms and intake documents. Consent forms ensure clients are fully informed about the services they will receive, including the nature, purpose, and potential risks and benefits. These forms address confidentiality, the use of personal information, the client's rights, and my practice policies. The inake documents are important for gathering the necessary information I need to prepare for your intake appointment.

  • What to Bring

    It is important to bring the following documents to your intake appointment.

    1. A copy of your most recent physical or well child check. If one has not been completed in the past year one will be required to complete the assessment.

    2. A list of any medications you are currently taking.

    3. Copies of any diagnostic assessments, occupational therapy evaluations, or speech therapy evaluations that you have completed.

    4. Copies of any special education evaluations and/or individualized education plans (IEPs).

  • Cancellations

    I understand that situations can arise unexpectedly, necessitating the need to cancel or reschedule an appointment. If you need to cancel or reschedule, please use the client portal you accessed to schedule the appointment to cancel or email me at revel.weber@infinitytherapyllc.org. Please cancel a least 48 hours in advance to avoid a cancellation fee. This policy allows me to offer the appointment slot to other clients in need of services. I appreciate your cooperation and understanding in helping me achieve a smooth and efficient scheduling process.

    During your intake appointment, we’ll take time to get to know you and understand your unique strengths, challenges, and goals. Whether you're seeking support for yourself or a loved one, we'll explore your needs in a safe and accepting space. We’ll review your history, discuss any current concerns, and start to develop a personalized plan for therapy. This first meeting is a collaborative process, allowing us to create a path that aligns with your individual journey. We are here to listen, provide insight, and support you as we embark on this therapeutic process together.

  • After the intake

    After the intake appointment, we’ll begin working on your personalized therapy plan. Together, we’ll set specific goals based on your needs and what you want to achieve in therapy. Each session will build on the last, focusing on your progress, addressing challenges, and exploring new strategies for growth. Along the way, we’ll regularly review and adjust your plan to ensure it continues to support your evolving goals. This ongoing process is tailored to your pace, allowing for flexibility and a responsive approach that honors your unique experiences.
